Consistent notes can make conversations between carers and vets more useful.

Knowing an animal's usual behaviour helps carers recognise when something seems different. A simple record can note appetite, drinking, movement and behaviour, alongside the date and the person making the observation.

Describe what you actually saw rather than writing a diagnosis. Share relevant changes with the responsible carer and follow the animal's existing veterinary plan. Keep records in a place the next person can find.

If an animal appears unwell, injured or distressed, contact your vet promptly. Recording observations must never delay getting help. This article gives general record-keeping ideas and does not define clinical thresholds or treatment.
